• Participant requires treatment
• Participant eligible for inclusion in the trial
• Clinician willing to accept randomisation
• Participant consent is obtained
• Participant formally entered on the trial
• Treatment assignment obtained by randomisation
• On-study forms are completed
• Intervention commences
